Medically Supervised Weight Loss
Tell us a little about yourself and our clinic team will reach out to get you started.
SMS consent is captured on the clinic signup form shown above. Every consent element follows these rules:
Appointment Reminders and Service Messages (Optional)
I agree to receive SMS text messages from Koumanis Diet and Health Clinic, including appointment confirmations, appointment reminders, scheduling changes, telehealth notifications, and customer-service messages. Message frequency varies. Msg & data rates may apply. Reply STOP to cancel, HELP for help.
Promotional Messages and Product Offers (Optional)
I agree to receive recurring promotional SMS text messages from Koumanis Diet and Health Clinic, including supplement offers, digital health platform promotions, Meal Planner offers, discounts, and information about clinic services. Messages may be sent using automated technology. Message frequency varies. Msg & data rates may apply. Reply STOP to cancel, HELP for help.
Patients can opt out at any time, and every message we send includes the required opt-out language.
Cancels the applicable SMS program immediately. After a STOP request, we send a confirmation message acknowledging the unsubscribe, and no further messages are sent through the opted-out program unless new valid consent is later provided.
Returns assistance information with our support contact details. Patients may also request help opting out by contacting our support team directly.
All messages identify our brand and include HELP and STOP instructions, consistent with the disclosures at opt-in.
Appointment Reminder (Transactional)
Telehealth Notification (Transactional)
Promotional Message (Marketing β separate opt-in required)
Providing a mobile telephone number alone does not constitute consent to receive promotional text messages. Marketing SMS consent must be separately and explicitly granted via the optional checkbox above.
